Consent for treatment is given to Daisy Bassen, M.D. Treatment may include evaluations and assessments, medication management, individual or family psychotherapy, or consultation with outside agencies and other treatment providers. The necessity for particular forms of treament will be discussed prior to beginning treatment, as well as while treatment is in progress. The patient agrees to provide copies of relevant past treatment, including neuropsychological testing, school reports, and records of hospitalization, to help Dr. Bassen provide the best possible care.
Dr. Bassen accepts Blue Cross and United Insurance (and, as of now, Harvard Pilgrim since they are responsible for United's Behavioral Health claims). She also accepts fee-for-service and will provide receipts for other insurance plans. An initial intake is billed at $350 and a 20-minute follow-up appointment is $125. Payment for sessions is due at the time of service. If the patient chooses to use insurance, it is understood that all co-payments are due at the time of service. It is understood that meetings which Dr. Bassen attends (e.g. school, collaborative meetings with other agencies) will be billed at the same rate as a clinical hour. Many insurance companies will not authorize payment for collaborative/collateral work, and therefore the cost is the patient's responsibility. Such types of collaborative/collateral work will not be done without the patient's specific permission and request, and it is understood that the patient is ultimately responsible for all fees incurred in treatment.
Patients must call at least two (2) business days before the scheduled appointment to cancel and/or reschedule.
Patients under the age of 18 must be accompanied by a legal guardian or they may not be seen.
The fees for missed appointments are:
- $30 for a cancelled follow-up appointment inside the two business day window
- $75 for a missed follow-up appointment with no notification
- $125 for a cancellation or no-show of an initial intake appointment
These fees are not billable to insurance, and patients are solely responsible for their payment. There is no fee for appointments missed due to emergency. As long as appointments are rescheduled, medication refills will be provided regardless of the reason for cancellation.
Follow-up appointments are scheduled for 20 minutes. If patients arrive more than 10 minutes late for a scheduled appointment, that appointment may be rescheduled and they may be charged a $30 fee for a missed appointment. If possible, you will be rescheduled for another appointment that same day.
